Admission to Sunnyside Christian School is by formal application.  We encourage you to request a tour and an application packet from the school.  You can also download application forms in PDF format.

To complete the admission process, the following is required:

1. Completed application for admission - only one per family needed.
2. Student information form - one per student needed.
3. Pastor's letter of recommendation submitted directly to the school.
4. Parental form of agreement.
5. Student reference form - for application to grades 6-12 only.
6. Complete immunization form.  

For pre-school use this Application.  All others use the application packet below.

 Immunization Forms  are available at the elementary school office.

When the application is complete, you will be contacted to arrange an interview time with the admissions committee. A parent (preferably both) and the student (if in grades 6-12) are required to attend.
